ISO: help with bread machine - bread heavy with hard crust

I have a Black n Decker all in one horizontal breadmaker. When I follow directions for a basic bread, it comes out heavy, and the crust hard as a rock. It doesnt' seem to be rising. Any troubleshooters out there? thanks
