I just tried the dish today myself and was looking for the recipe. I found only the listed ingredients - not the amounts - on a Houlihan's website announcing a new low carb menu. They are: mashed cauliflower, butter, cream cheese, and roasted garlic. Of course the dish is twice baked with a topping of cheddar cheese, real bacon bits, and shallots. I can't wait to experiment.
