ISO: Mayberry cookbook chocolate chip cookie recipe

Misplaced my Mayberry cookbook while moving! Now I need the recipe for Supreme chocolate chip cookies which was included. I always left out the nuts and M&M's and added a pinch of cinnamon to the recipe. They were addictive!!! Can anyone give me the basic recipe? Thanks.
