ISO: Why are my cookies flat and spreadout instead of fluffy?

Misc.
When I make chocolate chip cookies, sometimes, the cookies are flat and spreadout. Other times they are fine. I chill my cookie dough before baking and use cold pans. Is it the amount of butter or flour? One of the nagging questions I seem to have.
