ISO: Applebee's Sizzling Apple Pie w.ice cream

Hi, it's me again. Listen: I just realized that NY sent me the ingredients to this recipe. I really wanted to know how to put it together and bake it and for how long?.......what degree oven? how do you make a streusel sauce? how many apples? how much dough? etc.
Would appreciate this from anybody, thanks in advance.
