ISO: Canning Tomatos

In all my 50 years of canning tomato's, I have never heard
of this problem, My son was canning tomato juice,it was
seedless with no pulp] The jars were sterilized and filled
to 1/2" of the top, lids were hand tightened and placed in
pressure cooker at 10 pounds presure for 15 min. when he
opened the cooker, the lids had pushed out and open!! Can
someone tell me what happened ?
