ISO: Fix for Watery Sauce

So lets see here, I make my own red sauce with the following recipe:

Roma Tomatoes blanched and peeled and chopped
Fresh Basel and Oregano finely diced
Diced White Onion & Garlic sweated (so nice smelling)
Sweet Italion Sausage and lean Ground Beef
Salt, Pepper, Red Pepper, Sugar, a Perfect Zin

I bring the tomatoes to a boil and add sugar and the fresh Herbs. Then I add the Onion, Garlic, Meat of choice and simmer for hours. However when I apply the sauce to pasta or Chicken Parmesan and it cools the sauce itself seperates and leaves a pool of water. Very not what I was after. Can any self inspired cook provide me with a good solution to this problem?

Anxiously awaiting an answer for my next batch of heaven.
