ISO: johnny carinos chicken milano

I am looking for a recipe similar to the chicken milano that Johnny Carino's makes. It is different from the traditional chicken milano. This one has a white creamy cheese sauce, fresh sauteed mushrooms, ham, and obviously chunks of chicken over egg noodles or a similar wide noodle. we live two hour away from the nearest Johnny Carino's and having to wait until we go on a trip to enjoy this dish is cruel and unusual punishment (Ha-ha). Your help would be grately appreciated.
