ISO: noodles & company Wisconsin Mac & Cheese

I am looking for the recipe for noodles & company mac and cheese. I know it has cream and is toped with cheddar jack cheese but a restaurant employee told me it has three cheeses.

Does anyone know the actual cheeses used, and of course the recipe?

Would really like to make this at home.
