ISO: Spinach Artichoke Dip

I have been looking for a Spinich Artichoke Dip that has the following ingredients in it I just need to know how much of each ingredient.
Artichoke
Spinich
Swiss cheese
Chedder Cheese
Parmesan Cheese
Cream
Onions
Garlic
These are the ingredients I believe that are in the dip at the Damons Restraunt. Its soooo good!!!
