Bread machine blues

Misc.

The last three loaves made from my machine were a flop, literally. I am using
fresh bread flour, fresh yeast and separating the liquid and salt from the
yeast. All loaves have not risen properly and the loaf looks deformed. The
bread is not airy and some is uncooked. Is this my bread machine or me?
