ISO: Chicken Tetrazzini

Back in the 70's I had a copy of a recipe for Chicken Tetrazzini with at least 5 dairy products. It was high in fat but out of this world with milk, cream, butter, and cheeses. I lost the recipe & have searched for YEARS. Anyone have this?
