ISO: Looking for Cheese Omelet recipe w/Velveeta, Ritz Crackers, Evap. Milk ?

My mother made Cheese Omelet, but I don't have the recipe from at least 40 years ago -- the Velveeta was cubed, the Ritz crackers crushed, and I think it had evaporated milk and ??? She baked it in a casserole dish and it was one of our favorites! Does anyone have this heirloom recipe?
