ISO: Mom's Fudge

HELP!!!
When I was younger, my mother used to make a fudge that was shiny and hard and DELICIOUS. My mother is no longer with us and my brother keeps asking me to make it for him. I do remember that occasionally my mother would have difficulty with the fudge and I am also having trouble. It doesn't harden but turns out like taffy or carmel consistency. I don't know if I'm undercooking it or overcooking it. Am I supposed to stir it or not? I hope someone can help. The recipe is as follows:

2 cups of sugar
1/2 can evaporated canned milk
1 stick of real butter
1 square of unsweetened chocolate
1 Tsp. Vanilla

Bring to a full boil (I'm not sure what that is)for 4/5 minutes. Turn off heat add vanilla. Put pan in cold water w/ice (I'm not sure for how long) before pouring into pan.

I would be eternally greatful if someone can help me.
