Recipe(tried): Moolatte like Dairy Queen

Beverages
I worked at a DQ and we took a 16 oz cup and filled it about 1/3 of the way with our Misty Ice (the ice stuff we use to make artic rushes, formerly mr. misty's). Then added 3 squirts of triple strength coffee concentrate.

Then for whatever flavor you wantd: vanilla, we added 4 pumps vanilla syrup(same as used for shakes),the caramel we used was caramel fudge topping, mocha used cocoa fudge, cappaucino was just the coffee, and the new mint mocha was chocolate syrup and a peppermint syrup. That was mixed with ice cream and blended and topped with whipped topping and garnishes...

NOTE: One time we ran out of coffee concentrate and I made a temporary flavoring. it was triple strength so I added 3 times the amount of coffee we used to make a pot of coffe... Brewed the coffe then refridgerated it.. I wasn't sure if it would work, but I used it in the moolatte and it tasted the same to me. Hope this helps you out!!
