ISO: Layered Cookie Recipe (in a Jar)

Misc.
A while back there was a set of recipes posted for a great gift giving
idea. It was the ingredients for layered cookies (kind of like a soup
mix in a jar) that you attach the directions to and give as a gift. Does
anyone know where I can find it?

I didn't think to print it at the time and have a few people that would
really enjoy it.
