ISO: Make-a-Mix Hot cocoa mix

Misc.
I have misplaced my Make a Mix Cookery Cookbooks and would like to make up a batch of hot cocoa mix to send to our son in college. This would mean I would have to make it tomorrow and I have already wasted half the evening looking.
The recipe calls for

dry nonfat milk
powdered coffee creamer
confectioner's sugar
and
UNSWEETENED cocoa

I just don't remember the amounts.
Can anyone help??? Please?
