Recipe(tried): Dense Bread Machine bread - A few more things to check

Since you have tried everything else here are a couple more things to check:

Bread machines warm the dough during the rise cycle. Maybe it's getting too warm (over 110 degrees F) and killing or affecting the yeast. I would take the temperature of the dough toward the end of the rise cycle to confirm it's okay.

Tap water with too much chlorination can also affect or kill the yeast. Try using bottled water.

Is the rise cycle long enough? Maybe the timing of the rise cycle is not long enough if the machine is having trouble.

Have you tried yeast from another source? Maybe you have defective yeast.
