Try dough cycle and rise and bake outside the machine as test

If nothing else works, as a final test, I would use the machine on the dough cycle to mix and knead the dough. Then remove the dough and allow it to rise outside the machine and bake it in the oven. This would test to see if its the recipe/ingredients or the machine that is causing the problems.
