ISO: Ultimate Macaroni and Cheese

I lost my recipe for macaroni and cheese that I found in an issue of Bon Appetit. It uses dry mustard, cayenne pepper, Worchestershire, half-and-half and whipping cream. It was titled Ultimate Macaroni and Cheese and I think it was a restaraunt recipe. Can anyone find it for me?
