ISO: Hershey's Cocoa Fudge - help me please

I've been tring to make this for the last five years. It never turns out for me. Can someone help me and maybe let me know what I'm doing wrong. We have had this recipe forty years or longer. My grandma use to make it for me and I can't seem to get it right. please help me.
