ISO: Homemade Nescafe Ice Java coffee syrup

We love this stuff in our milk, but Costco doesn't carry it anymore. The smaller bottles are
about the same price as the jumbo bottle Costco carried. Does anyone have an idea how to make this stuff? It has chocolate in it, too, giving it a mocha flavor.
