ISO: hersheys cocoa fudge

I made this recipe with eveporated milk and it did not came out good.It didn't even set right. Helpppppppp. Does yours always come out? I think this is the one my mom use to make when my kids were young. It was the recipe on the back of the can.Please help me figure out what I did wrong.
